Medical History and Lifestyle Questionnaires
Initial appointments with Dr. Finkelstein are generally scheduled for 1-1/2 to 2 hours.

If you are scheduling an appointment with Dr. Finkelstein it is suggested that you complete the appropriate Holistic Medical Evaluation Intake Form and the Wellness Self-Test (only for adults), see Forms below, and mail (please do not fax) a week prior to your first appointment. Giving Dr. Finkelstein an opportunity to review this information before your appointment will help make your first session more focused. It may also help you organize your thinking and approach to complex issues that contribute to your current state of health. Keep in mind that it is not necessary for you to answer every question, but you are encouraged to take some time to thoughtfully complete this and mail it to SunRaven so that it arrives at least a few days before your appointment.

You should also download the registration form and send that as well.

Payment Policy
Scheduled appointments can be paid for in advance of the actual appointment date, through the appropriate PayPal link. Alternatively, payment by cash, check or credit card at the time of the appointment will be accepted. If you intend to pay by credit card you should also download the credit card authorization form and complete it with signature and fax it to SunRaven at the time of booking your appointment. Cancellation Policy
If you need to reschedule/cancel your appointment please make every effort to do so as soon as possible but no later than 72 hours prior to your appointment.

Last minute cancellations have great impact on the scheduling options for others. Cancellations made less than 72 hours prior to your appointment will be billed to your credit card at half the fee of your scheduled appointment.
